It depends on the severity of the sagging. There is no rule that says you can't have one at 38.
I would go to the appointment with an open mind, see what the doc says and then perhaps get a few other consults lined up for different opinions. Then you can weigh up the pros and the cons before going ahead with any surgery. There may even be alternatives such as fillers to the upper face, which can give the illusion of a cheek lift.

Looking at your picture, I really don't think you need any kind of lifting just yet. You still have a nice youthful contour to your face. I know we can be so hard on ourselves sometimes, but you are going to look back on pictures in 5-10 years and wonder what you were worried about. I would definitely wait a while and in the meantime just enjoy life.
Also I have heard of people having complications with endotine lifts, it's just another one of those techniques that doesn't live up to the hype. And whatever you do stay away from threadlifts. Sometimes doctors don't have your best interests at heart, all they see is $$$'s.
